Info/Application for Non-PCSD Home/Charter/Community School Students to Participate at PSH

Per Ohio Revised Code, Home School/Virtual/Charter/Community School Students whose school does not offer an opportunity to play the sport at their home school are allowed the opportunity to participate in the athletic program at the public high school in their residential attendance zone.   In order to do this, an application must be filled out and submitted to the Athletic Director prior to the start of the season.  A link to the application can be found here:

Along with the application, the parent/guardian must provide the following:

  1. Proof of Home Residency* (Mortgage Statement/Utility Bill etc)  *Must live in the Parma Sr. High Zone of the PCSD
  2. Proof of Age (Birth Certificate)
  3. Proof of Education Status (Most Recent Report Card)

An approved application allows the Non-PCSD student the same opportunity as the regular PCSD student and they must follow the same rules, regulations and expectations including OHSAA/PCSD academic eligibility requirements, PCSD/Team behavioral expectations and team roster selections.  In sports that have roster reductions (“CUT” Sports), the opportunity to try-out is all that is guaranteed.

Parents must also register their student-athlete on “Final Forms” and fill out and sign the online paperwork on the Final Forms website. A link to Final Forms can be found on the main menu under “More.”      

The athlete must submit a valid OHSAA physical exam form filled out and signed by a medical doctor.  A blank physical form can be printed off:  Physical form_2020-21_ 

Physicals are valid for one calendar year from date of exam and must get the athlete to the completion date of the season.
